Here’s What You Can Expect From College Square Dental:
Scheduling
Call or email us to book an appointment at a time that best fits your schedule. We offer evening appointments.
First Appointment
Before your first appointment, you’ll be provided with a digital medical history form to complete. The complete exam at a first appointment will evaluate your oral health holistically as a part of your overall health.
We’ll evaluate all of the information collected and present the treatment options available to you along, with the benefits and limitations of each.
Insurance Claims & Assignment of Benefits (AOB)
At College Square Dental, we’ll help make insurance easier by submitting claims on your behalf whenever possible. Some plans allow an Assignment of Benefits (AOB), which authorizes your insurance company to send payment directly to our office.
While we’re happy to assist, insurance coverage is determined by your plan and insurer. Estimates are based on the information available at the time and are not a guarantee of payment. You are responsible for any portion not covered by insurance (including deductibles, co-payments, frequency limits, or services your plan does not cover). If your claim is denied, reduced, delayed, or paid directly to you, any remaining balance is payable.
